It has a serious complication of excessive bleeding – and no known antidote. Unlike Warfarin – the go-to blood thinner for more than 50 years – Xarelto has no antidote to stop its anti-clotting action in an emergency.

Xarelto is a prescription medication used to reduce the risk of stroke and blood clots in people with atrial fibrillation not caused by a heart valve problem.  Xarelto is the second FDA approved drug in a new generation of anticoagulants that are marketed as alternatives to Warfarin. Unlike Warfarin, Xarelto is an easy-to-use pill taken 1 to 2 times a day.  The most concerning problem with Xarelto is that it has no effective reversal agent for excessive bleeding– unlike Warfarin which can be reversed with a dose of Vitamin K.

Xarelto was orginally used to reduce the risk of forming blood clots in people who have just had knee or hip replacement surgery. Now it is also used to treat Deep Vein Thrombosis and Pulminary Embolisms to reduce the risk of these conditions occurring again.

The complications can also vary for patients by use. For people taking Xarelto for atrial fibrillation they include:

  • Are at increased risk of forming a blood clot in the heart, which can travel to the brain, causing a stroke, or travel to other parts of the body
  • Lowers your chance of having a stroke by helping prevent clots from forming
  • If you stop Xarelto, you have an increased risk of forming clots
  • CAN CAUSE BLEEDING which can be serious, and rarely may lead to death
  • Can bruise more easily
  • Will take bleeding longer to stop

You are also at a higher risk of bleeding if you take Xarelto while also taking the following:

  • Aspirin / aspirin containing products
  • Non-steroidal anti-inflammotory drugs (NSAIDs)
  • Warfarin sodium (Coumadin®, Jantoven®)
  • Clopidogrel (Plavix®)
  • Other medications to prevent or treat blood clots
  • Nizoral/ Ketoconazole
  • Itraconazole (Onmel, Sporanox)
  • Ritonavir (Norvi)
  • Lopinavir/ritonavir (Kaletra)
  • Indiavir (Crixivan)
  • Carbamazepine (Carbatrol, Equetro, Tefretol, Tefretol-XR, Teril, Epitol)
  • Phenytoin (Dilantin,Dilantin-125)
  • Rifampon (Rifater, Rifamate, rimactane, Rifadin)
  • John’s wort

Warning signs for anyone taking Xarelto that should be immediately reported to your doctor:

  • Unexpected bleeding or bleeding that lasts a long time, such as:
    • Nosebleeds
    • Unusual bleeding of gums
    • Menstrual bleeding that is heavier than normal; vaginal bleeding
  • Bleeding that you cannot control
  • Red, pink or brown urine
  • Bright red or black stools
  • Cough/vomit up blood or blood clots
  • Vomit looked like “coffee grounds”
  • Headaches, dizzy or weak
  • Pain, swelling or new drainage at wound sites
  • Spinal or epidural blood clots (hematoma)
  • Not for patients w/ artificial heart valves
